Intake Specialist - Providence House

Position Summary

As an Intake Specialist, the successful candidate will coordinate services and conduct intakes and discharges in conformance with the agency's program with the Cuyahoga County Division of Children and Family Services (CCDCFS). The successful candidate will excel in managing the waiting list, assessing the needs of the clients, and maintain a collaborative relationship with CCDCFS.

Essential Duties and Responsibilities

Oversees the intake and discharge process for the CCDCFS program. This includes screening inquiry calls, validating referrals, managing the waiting list, and completing all qualified intakes for the beds contracted to be utilized for CCDCFS. Completes referrals, coordinates transportation for children's school drop-offs and pick-ups and appointments, and ensures all medical care is provided for children in the CCDCFS program. Completes intakes, case management services, visitation coordination, and all required documentation for respite families admitted to either Elisabeth’s House or Leo’s House in conjunction with the Family Services Coordinator. Works directly with families to determine frequency and scheduling of visitation if applicable. Assists in scheduling and completing outreach appointments with community partners, CCDCFS and referring agencies as assigned. Assists in leading internal training sessions that addresses staff understanding of the psychosocial needs of clients served. Assists with the execution of Providence House internal and external special events for the organization. Strives to be culturally competent and supportive of diversity in both service to clients and through professional interactions.

Required Qualifications and Preferred Skills

Minimum of Bachelor’s Degree in Social Work; Master’s Degree in Social Work preferred. Must be a Licensed Social Worker (LSW) in the state of Ohio. 1-3 years of experience with intake coordination preferred. Must be 21 years of age. Must have a valid Ohio driver’s license. Must be able to travel in the Northeast Ohio area and have reliable transportation. Must be able to accommodate weekend and evening responsibilities as needed.

This is a full-time, exempt position. The candidate must be available to work a flexible schedule based on agency needs including evenings and weekends as needed.
